Safe and Supported at Home (SASH)
Short-term help to stay safe at home while you organise longer-term supports.
The Safe and Supported at Home (SASH) program provides time limited, practical home supports for people with functional impairments. It helps prevent avoidable hospital admissions and supports safe discharge home.
Who it’s for
- Adults who have functional impairments and need help at home.
- People who are in the process of applying for longer term disability or community supports, or who have been found ineligible.
- People leaving hospital or at risk of an avoidable admission (referral usually via hospital or Local Health District).
Eligibility criteria are set by NSW Health/your Local Health District. We’ll help you understand the pathway.
What’s included (examples)
- A dedicated case manager to coordinate your supports.
- Domestic assistance and personal care.
- Meal preparation and nutrition support.
- Transport to essential appointments.
- Shopping, errands and safety checks.
- Short term respite for carers.
How it works
Referral: Usually comes from your hospital team or Local Health District.
Quick start: We organise the right mix of supports to help you stay safe at home.
Review: We check in and adjust as your needs change.
Duration: Up to six weeks, depending on your needs and program settings.
Costs: Funded by NSW Health. A small client contribution may apply depending on Local Health District guidelines. We will explain any fees upfront.
