Practical help so home feels safe after hospital.
ComPacks provides short-term, coordinated supports when you’re discharged from hospital.
Typical supports:
- Domestic assistance and meal support.
- Transport to medical appointments.
- Shopping and errands.
- Personal care and safety checks.
- Carer support and respite.
How it works
Referral: Usually arranged by your hospital team.
Quick start: Services begin soon after discharge.
Review: We adjust supports as your recovery progresses.
Duration: Short-term (weeks), based on your needs and program settings.
Costs: May be funded or subsidised; if contributions apply, we’ll explain them clearly.
