Out-of-Hospital Care  

Short-term help after hospital or during serious illness.

If you’re leaving the hospital or managing a serious health condition, short-term home supports can help you recover safely and avoid readmission. We coordinate closely with hospitals, GPs and palliative care teams. 

What’s available? 

Community Packages (ComPacks) – short-term, practical support after a hospital stay.

Safe and Supported at Home (SASH) - short-term support for people with functional impairments after a stay in hospital.

End of Life (EoL) supports – person-centred help at home alongside clinical care.

How to access

  • Hospital referral: Many programs start with a referral from your hospital or clinician.
  • We coordinate: We’ll arrange services quickly and keep everyone informed.

These programs focus on safe recovery and comfort. They complement, not replace, clinical care.
