How home care funding works 

Home care funding helps people stay living at home. Depending on your age and situation, there are different programs:  

  • Aged Care programs – for people aged 65+ (or 50+ for Aboriginal and/or Torres Strait Islander people). This includes entry-level support through the Commonwealth Home Support Program (CHSP) and higher-level support through the Support at Home program. 
  • Disability programs – for people living with disability. The main pathway is the National Disability Insurance Scheme (NDIS). 
  • Out-of-Hospital Care – short-term supports after a hospital stay or during illness (e.g. Community Packages/ComPacks, Safe and Supported at Home packages and End of Life supports coordinated with NSW Health).

Getting assessed for government-subsidised home care

When you’re ready to get support at home, the first step is a quick eligibility assessment. It points you to the right program and helps match services to your needs. 

Which pathway do I use?

My Aged Care (Support at Home/CHSP): If you’re 65+ (50+ for Aboriginal and Torres Strait Islander people) or supporting an older family member, start here. The online form takes about 15–20 minutes.

NDIS (for disability): If you’re under 65 and live with a significant, permanent disability, check your eligibility and apply through the NDIS
